SNAP Care is looking for a highly skilled Support Professional to join their team and provide expert care and support for a 10-year-old child with cerebral palsy.
Responsibilities:
• Develop and implement personalized plans to promote the child's independence and well-being.
• Collaborate with the child's family to ensure a smooth transition between home and school.
• Facilitate participation in activities that cater to the child's interests, promoting socialization and emotional well-being.
The successful candidate will have experience working within a family setting, supporting young individuals with physical disabilities, and possess excellent interpersonal skills.
A full UK driving license and over 21 years old are essential requirements for this role.
